If your parent or spouse served in the U.S. military, they may have earned a benefit that many families overlook: the VA Aid & Attendance pension. This monthly, tax-free payment can help wartime veterans and their surviving spouses pay for the cost of long-term care — including care in an assisted living community.
It's one of the most underused benefits available to aging veterans. Below, we explain what Aid & Attendance is, the 2026 benefit amounts, who qualifies, and how to apply.
Aid & Attendance (A&A) is an enhanced monthly amount added on top of the basic VA pension. It's intended for veterans (and surviving spouses) who need help with everyday activities — bathing, dressing, eating, managing medications — or who are largely confined to their home or living in a care setting.
Because assisted living provides exactly this kind of help, A&A is frequently used to offset the cost of an assisted living community.
For the period running December 1, 2025 through November 30, 2026, the Maximum Annual Pension Rates (MAPR) with Aid & Attendance are:

Here's the key thing to understand: the benefit pays the difference between the household's countable income and the MAPR. And because unreimbursed medical expenses — including assisted living and care costs — can be subtracted from income for VA purposes, many families who assume they "make too much" actually qualify once care costs are factored in.
There are four general requirements:
Can my mother get Aid & Attendance as a surviving spouse? Yes. A surviving spouse of a qualifying wartime veteran who has not remarried may be eligible, provided the care-need, income, and net-worth tests are met.
Does the veteran need a service-connected disability? No. Aid & Attendance is a pension benefit based on wartime service and financial/care need — it is different from service-connected disability compensation. The disability does not have to be related to military service.
Can we use Aid & Attendance and Medicaid together? The two programs interact in specific ways, and Florida Medicaid counts VA income differently depending on the situation. How long does approval take? Often several months. Benefits are frequently paid back to the filing date, which is why families shouldn't wait to apply.
